Evidence-backed extraction
Checksum-bound fixtures, validated output and original evidence spans keep every derived observation inspectable.
One case · Inspectable evidence · Governed decisions
When a reported brake fault repeats and becomes safety-critical, what evidence supports holding the asset from service, and who authorizes the decision?
Follow six synthetic reports through an evidence-backed review, a deterministic repeat-event rule and a recorded approval. Inspect the implementation behind each step before considering a workflow pilot.
Authored synthetic reports. Their dates describe the example history, separately from the later application capture.
The maintenance CSV reports brake pad replacement and a responsive pedal after repair. Completion belongs to this earlier work order, not closure of the later case.

The vague spring repair reference carries authored 70% confidence and its original passage. The review queue shows it pending; the scripted visitor accepts it through the real UI, and an observation-accepted audit records the transition.

The predicate requires a safety-critical severity observation AND at least two related events within 1,440 hours (60 days). The related-event aggregate counts related events; it does not require every counted event to be safety-critical. The displayed resolved count is captured from the real rule trace.
The Rule Inspector displayed its latest matched evaluation. This does not identify the initial evaluation that first created the idempotent decision proposal.
Embedded support: approval-rule
The matched rule proposes a critical-risk remove-from-service decision under the Asset Removal Approval policy. The pending export establishes its awaiting-approval state before the scripted visitor acts.
Embedded support: approval-rule
The approval dialog disables confirmation until a comment is supplied. The synthetic script supplies an explicit demonstration comment. The Approval, Decision and approval audit must share consistent references, outcome and time.
Selected export projection with public aliases; this does not independently verify the original audit hash chain.
The final export supplies the open case and actual action-item states. No action completion, repair outcome or business benefit is inferred from the approved Decision.
Synthetic scripted review and approval; no maintenance lead was consulted. No physical execution or measured business impact is established.
Each capability below has implementation or verification evidence at the recorded application revision.
Checksum-bound fixtures, validated output and original evidence spans keep every derived observation inspectable.
A pending field is accepted through a session-scoped UI action that preserves review history and appends audit evidence.
A pack-owned predicate evaluates severity and related-event history separately, with the resolved aggregate visible in the rule inspector.
The application persists a distinct Approval before updating a Decision; capture verifies that linkage and preserves outstanding case work.
Scenario Packs supply domain vocabulary, schemas and policy while the shared lifecycle and export services remain neutral.
This file describes the recorded revision. Later publication or repository changes do not turn it into evidence about newer code.
These hashes identify inputs used by the capture. Later harness changes do not rewrite the historical recording.

[Northgate Facilities Maintenance Chat - #zone-c-shipping] Dana Osei, 2026-05-18 09:14 something's off with A-142's brakes today. getting a grinding noise when I brake hard, and the pedal feels spongy, like it's got more travel than usual before it actually grabs. noticed it a couple times this morning moving pallets near the shipping doors. Dana Osei, 2026-05-18 09:15 it's still stopping, just doesn't feel right. wasn't like this last week. I know it had brake work done back in the spring so figured someone should take a look before it gets worse Dana Osei, 2026-05-18 09:16 going to keep using it carefully for now but wanted to flag it early

--- page 1 --- NORTHGATE FACILITIES Follow-Up Equipment Inspection Report Asset: A-142 (Talbrook TF-40 electric forklift) Location: Zone C - Shipping Inspection Date: 2026-05-21 Inspector: Lena Fischer, Northgate Facilities Purpose: Follow-up inspection prompted by operator-reported brake grinding noise and spongy pedal feel (reported 2026-05-18). Background: Unit had a full front and rear brake pad replacement performed by Ferrow Industrial Services on 2026-04-06 (work order FIS-4471) following a similar soft-pedal complaint at that time. This inspection is intended to determine whether the current complaint represents a recurrence or a separate issue. --- page 2 --- Findings: Brake pedal travel is noticeably longer than spec before engagement, consistent with the operator's description. Some grinding is audible during hard braking, most pronounced at low speed. Visual inspection of the brake assembly shows the recently replaced pads are wearing unevenly - more material loss on the inboard side than expected for six weeks of service. There is also a small amount of fluid weeping visible at a fitting on the hydraulic brake line near the left rear wheel, not enough to call it an active leak yet, but enough to note. Given that this is the second brake complaint on this unit in under two months, and the pad wear pattern is not consistent with normal use, this looks more like a recurring issue than an isolated one - possibly a sticking caliper or a slow line leak causing uneven pad wear and reduced pedal firmness. Recommend a more thorough diagnostic beyond a simple pad swap. --- page 3 --- Recommendation: Follow-up diagnostic and repair should be scheduled within 5 business days (by 2026-05-26) given the safety-relevant nature of a braking system and the recurrence pattern. Unit may continue in service in the interim only with caution and should be pulled immediately if the operator reports any loss of braking effectiveness, not just noise or feel. Inspector signature: Lena Fischer Report filed: 2026-05-21 --- page 4 --- [End of report]

[Northgate Facilities Maintenance Chat - #yard] Owen Vasquez, 2026-06-02 14:37 need someone from maintenance NOW. was bringing A-142 up the yard ramp with a load and the brakes did not hold, it started rolling backward and I nearly rolled into the rack at the bottom before I got it stopped with the parking brake. that is not a "feels spongy" problem anymore, the brakes flat out did not hold on the incline Owen Vasquez, 2026-06-02 14:39 I pulled it and parked it at the bottom of the ramp, not moving it again until someone looks at it. this could have gone a lot worse, there were people near the bottom of that ramp Owen Vasquez, 2026-06-02 14:40 I know it had a follow-up flagged after the inspection a couple weeks back, this needs to jump the line

From: Priya Nandakumar <p.nandakumar@northgatedc.example> To: maintenance@northgatedc.example; safety@northgatedc.example Subject: URGENT - A-142 brake failure on yard ramp, requesting hold from service Date: 2026-06-02 Team, Escalating this immediately. This afternoon Owen Vasquez was operating A-142 on the yard ramp with a load and reported that the brakes did not hold, causing the unit to roll backward toward the storage rack at the bottom before he was able to stop it using the parking brake. No injuries and no contact, but this was close and it happened on an incline with people working nearby. This is not the first flag on this unit's brakes. For reference: - 2026-04-06: brake pad replacement by Ferrow Industrial Services after a soft-pedal complaint. - 2026-05-18: operator reported grinding and spongy pedal again. - 2026-05-21: follow-up inspection found uneven pad wear and fluid weeping at a line fitting, recommended a full diagnostic within 5 business days. - 2026-06-02 (today): brake failure to hold on an incline under load. Given the pattern and the severity of today's incident, I am formally requesting that A-142 be held from service effective immediately, pending a full review and sign-off before it goes back on the floor. Please do not return this unit to service without that review regardless of any quick fix that gets applied - I want the full history looked at, not just today's symptom. Owen has parked the unit at the bottom of the yard ramp. Please have maintenance move it under power-off/tow only if it needs to be relocated. Repair Update - Zone C / Yard Asset: A-142 (Talbrook TF-40 electric forklift) Date: 2026-06-03 Logged by: Marcus Ibe Picking this one up per Priya's email yesterday. Towed A-142 in from the bottom of the yard ramp this morning and got it into a bay in the Maintenance area. Pulled the wheels to get a look at the brake assembly before doing anything else. Assigning this to myself for the full diagnostic Lena's report called for. Not doing a quick turnaround on this one - given the ramp incident and the repeat history I want to trace the hydraulic line fully and check the caliper before anything gets reassembled. Ordered a replacement caliper and line fitting kit in case that's what we're looking at, should be in within a few days. Status: A-142 is OUT OF SERVICE, tagged and locked out. Not to be returned to the floor without sign-off per Priya's request. Awaiting parts and a decision on scope of repair before proceeding further.

# tour content. See docs/agent-runs/OIW-004b.md.
